This form is used to request the Veterans' Administration to furnish the Dutchess County Department of Human Resources with the applicant's medical and disability records in order to receive additional credits on a civil service exam as a disabled veteran. Required:

• 2 copies of Section I., typed or completed in ink
• Forward to the Regional Office of Veterans' Administration where the disability claim is on file.

Candidates who have filed applications for examination or employment, job reference forms or other information and/or whose names are on civil service examination eligible lists should file this form with the Dutchess County Department of Human Resouces in order to update their records with changes in personal information such as name, legal residence, mailing address, phone number, etc.

This application is used to apply for a civil service examination or job opening that has been announced. The candidate should read the examination or recruitment announcement prior to completing this application to determine the required minimum qualifications for the position. • Most examinations are announced 3 to 6 weeks before they are held. Completed applications may be submitted only during the announced filing period and must be submitted by the announced deadline
• Application must be signed
• Make sure that you meet the minimum qualifications indicated on the exam announcement before applying
• A non-refundable processing fee must accompany each application for an examination for the amount indicated on the examination announcement (no fee is required for applications for announced job openings)
• Make check or money order payable to Commissioner of Finance and include on the check:
       - your name
       - social security number
       - exam title
       - exam number

These may be completed and submitted at any time. Your name will be entered in our computerized job data bank for a period of one year, during which  time you may receive notification of examinations and vacancies for which you qualify .

• For candidates already discharged at date of examination, all documentation must be submitted prior to establishment of eligible list
• For Disabled Veterans' Credit, proof of disability from the Veterans' Administration (if proof has not already been submitted within the past year)
