Immunization Clinic Requirements

No appointments are needed.
Children under age 18 must be accompanied by a parent, legal guardian, grandparent, adult aunt /uncle or adult brother/sister.
Bring the child's immunization record to each clinic site.
Free vaccinations are available to persons 18 years of age and younger. Persons 19 years of age and older may receive measles-mumps-rubella (MMR) at adult clinic free of charge for college attendance. Proof of enrollment is required.
