Q.   How do I make a Freedom of Information (FOIL) request?

Simply write your request and send it to the “FOIL Officer” at the Dutchess County Water and Wastewater Authority’s main office.  Your request must reasonably describe the records for which you are looking and should mirror the format outlined in the sample request provided.  The following is the sample request letter: Sample FOIL Request (.pdf)

When an agency receives a request, §89(3)(a) of the NYS Freedom of Information Law requires that it has five business days to grant or deny access in whole or in part, or if more time is needed, to acknowledge the receipt of the request in writing and indicate an approximate date by which the agency will respond to the request.

Cost for records – The typical charge for records is twenty-five cents per page - up to 9”x14” size.  For paper copies which are larger than 9”x14”, and/or for records which may be in other formats/media, charges are based on actual production costs.  You will be advised of the estimated cost at the time the requested records have been identified and become available to you.

Q.   How can I obtain copies of my military service and discharge papers?

To obtain copies of your military records, you have two avenues open to you:


  • The Dutchess County Division of Veterans Services will assist you in obtaining your papers.  Contact them at (845) 486-2060 for more information (hours by appointment only).

  • You can order copies of your records from the National Archives and Records Administration; their website provides contact information and order forms.

Q.   How can I get a copy of someone’s will?

To obtain a copy of a will that was filed in Dutchess County, you should mail your request, along with the $30.00 fee, to:

10 Market St.Poughkeepsie, NY  12601

In your request letter, you may want to also ask for other information about the estate, such as when the will was filed and who the executor and the attorneys were.  For more information, contact the Dutchess County Surrogate Court at (845) 486-2235.

Q.   Where can I get certified copies of vital records such as birth certificate, marriage license, death certificate?

Either contact the municipality where the birth occurred OR visit the New York State Department of Health website and scroll through the instructions to learn how to place an order for a birth certificate copy via either the Internet, by fax, by telephone or by mail.

Q.   Where can I find the Dutchess County Department of Health’s fee schedules?

Our fee schedule contains fees for Engineering Review, Permitted Facilities, Tanning Facilities, and Environmental Laboratory Services.

Certain other services, including some immunizations, may have fees.  See the particular program for details.

Q.   Where can I find soil survey maps?

Q.   When and where do I file the OCFS (Office of Children and Family Services) claim forms for reimbursement?

Nonprofit agencies file the claims with the Division of Youth Services on a monthly basis.  Youth Development and Delinquency Prevention Programs (YDDP) may file quarterly.  Municipalities file directly to the News York State Office of Children and Family Services.

Q.   Where can I get claim forms for OCFS (Office of Children and Family Services) funding?

Claim forms are available in Excel and can be e-mailed, or hard copies can be obtained from the Division of Youth Services.

Q.   How do I apply for funding from the New York State Office of Children & Family Services (OCFS)?

Q.   What do I need to send to the Youth Bureau with my OCFS (Office of Children and Family Services) claim forms?

We require backup documentation for expenses claimed (i.e. payroll registers, copies of bills, receipts and copies of checks.)
